PLT Community Survey – Prevention Leadership Team

The DuPage County Prevention Leadership Team (PLT) is launching a community survey, which may take about five minutes to complete. Data collected through this survey will help the DuPage County Prevention Leadership Team (PLT) in making data-driven decisions, applying for additional grant funding, and better understanding the needs related to youth substance use and mental health in DuPage.
